There will be a 20 minutes snacks & bathroom break. Please pack a light snack and a drink, for each day of camp. Please refrain from bringing any snacks that contain peanuts, as a curtesy to any other participants that might be allergic to peanuts. FMS will not be responsible for any repercussions if participants share snacks. In case of any emergency, FMS will contact the responsible party's phone number provided above. If the responsible cannot be reached, FMS will contact 911.

I give permission to the Forté Music School LLC hereinafter named FMS, to photograph, record or video-tape me and/or my child(ren) during the course of taking lessons, recitals and other performances that are school sponsored, and to use the photos/video footage for purposes of promotion and archive. Parents, legal guardians of minors, students and adults waive the right to any legal action for any injury sustained on school property resulting from normal lesson activity or any other activity conducted by the students or their siblings before, during, or after lesson time, including hearing loss or damage. Children must wait for their parents to pick them up inside the school. Please do not ask them to meet you elsewhere. Students are not to be dropped off on the street or curb. The FMS assumes no responsibility for children who leave unaccompanied. The students are not to be left at the school for extensive time periods and the school is not responsible to provide before or after care for students.

I understand that the information on this form will be entered into the FMS MindBody online scheduling software and will be available to the FMS front desk staff & teachers only; it is not available to the public and will never be sold. Any personal information can be erased upon request. I agree to receive email and text communication regarding appointment changes, updates, news, events, etc. Any communication can be terminated upon request at any time.

Fees: Camp tuition needs to be paid in full at registration. We accept Visa, MasterCard, Discover, and American Express. No Cash or Checks.

Missed Classes: Tuition is transferable to another camp based on availability with minimum notice by Thursday prior to camp start date, but non-refundable. Cancellations after this time are non-refundable and non-transferable. Please be considerate and notify us in the event you are unable to attend your regular scheduled class.

Teacher Absences: We reserve the right to provide a substitute teacher if the regularly scheduled teacher is unable to teach any class (we will do our best to notify you in advance if a substitute will cover for your regular teacher). If a teacher is unavailable and we cannot provide a replacement teacher, the school will make a decision on when to schedule a makeup class.
