I give permission to the Forté Music School LLC to photograph, record or video-tape me and/or my child(ren) during the course of taking lessons, recitals and other performances that are school sponsored, and to use the photos/video footage for purposes of promotion and archive.
I hold employees/contractors of the Forté Music School and it's endeavors harmless from liability during the time I and/or my child(ren) is participating in any of the school’s programs. Parents, legal guardians of minors, students and adults waive the right to any legal action for any injury sustained on school property resulting from normal lesson activity or any other activity conducted by the students or their siblings before, during, or after lesson time, including hearing loss or damage. Forté Music School is not responsible for any damage done to vehicles or items stolen from vehicles.
Children must wait for their parents to pick them up inside the school. Please do not ask them to meet you elsewhere. Students are not to be dropped off on the street or curb. The FMS assumes no responsibility for children who leave unaccompanied. The students are not to be left at the school for extensive time periods and the school is not responsible to provide before or after care for students.
The credit card type/number that will be provided to Forté Music School for payments can be changed/updated at anytime before the 15th of the month. It is the student/parent’s responsibility to keep a current and valid card on file at all times while taking lessons with the school.
There is a $25 registration fee per family each time you enroll. Tuition is $100 per month for 30-minute lessons, and $200 per month for 60-minute lessons. Our calendar is designed to have an even number of 48 lessons per year, no matter which day of the week you choose, you will still end up with the same amount of lessons per year. Most of the time you will have 4 lessons each month, sometimes 5, and rarely 3 lessons per month, due to holidays. All of our programs are based on a monthly commitment. Payment is due on the first day of the month. If a student starts in the middle of a month, the remaining lessons are prorated.
Lesson Time/Day Changes:
If you need to change your lessons due to other commitments (going on with sports or other activities/interests) as always, we will try our best to accommodate you and find another time that works.
: Forté will only close on Snow Emergency Level 3, and when that happens, we will make sure to post on our web site, social media, as well as send notifications via emails & texts. In order to make up the entire day that was missed, we try to open on the exact same day/time that the lesson was missed, during a scheduled vacation day such as Spring Break or Summer Break.
: We are aware that the spring break vacation does not coincide with all schools. If your spring break does not coincide with Forté’s spring break, we will find a solution so you do not end up at loss.
During the summer months of May, June, July & August, we will allow a total of 6 make ups to be used anytime
during the summer months only
. That is an extra 2 make ups total, considering summer vacations.
Missed Lessons & Make-Up Lessons:
There are no credits or refunds given for missed lessons. Each student is allowed to claim a Make-Up Lesson for no more than
1 missed lesson per month
. It is the student’s responsibility to schedule a make up lesson with the teacher. Make up lessons will be honored ONLY if students give proper notice of their absence. Proper notice means at least 3 hours before your lesson for last minute changes/emergencies, or 24 hours in advance if a known absence is approaching.
No make up will be considered for a no call/no show.
Once a make-up lesson has been scheduled, no additional lesson will be offered if you cancel the scheduled make-up lesson for any reason.
Please be considerate when cancelling your scheduled lesson and give as much notice as possible. Good habits like this will give each instructor more time to consider make up lessons in their daily schedule.
Considerations for illnesses, extenuating circumstances etc. will be reviewed on an individual basis. Make up lessons must be completed within each semester: Spring (Jan, Feb, Mar, Apr), Summer (May, June, July, Aug) and Fall (Sept, Oct, Nov, Dec). If make ups are not completed within that time frame, they will be lost. Make Up lessons will only be granted to current students paying regular tuition, and they cannot be exchanged for monthly tuition.
Teacher absences will be covered either by a Substitute on the regular lesson day (we will do our best to notify you in advance if a substitute will cover for your regular teacher), or the teacher will make arrangements to do a pre, or post make up with each student.
All registered students are required to enroll in our auto-billing. This is a safe, secure, and convenient way to pay ongoing fees while enrolled at the school. We accept Visa, MasterCard, Discover, and American Express. We will automatically bill your card on the 1st day of every month.
15 days written notice - by the 15th day of the current month
to discontinue lessons. If no notice is given by the 15th day of the current month before lessons are being discontinued/suspended, then the student/parent will be responsible for the next full month’s tuition.
In case the card on file is declined, we have a late payment fee policy of $20. If payment is not received within the first week of the month it is considered late and the fee will automatically be applied. It is the students/parents responsibility to ensure we have a current credit card on file for the auto billing.
In the event a student needs to move on to the next level music book, or needs a special instrument accessory, with your approval, and for your convenience, we can place those charges to the card we have on file. We will always make sure to ask and get your approval before we charge anything.
