I give permission to the Forté Music School LLC hereinafter named FMS, to photograph, record or video-tape me and/or my child(ren) during the course of taking lessons, recitals and other performances that are school sponsored, and to use the photos/video footage for purposes of promotion and archive.

I also hold employees/contractors of the Forté Music School and its endeavors harmless from liability during the time I and/or my child(ren) is participating in any of the school’s programs. Parents, legal guardians of minors, students and adults waive the right to any legal action for any injury sustained on school property resulting from normal lesson activity or any other activity conducted by the students or their siblings before, during, or after lesson time, including hearing loss or damage. Forté Music School is not responsible for any damage done to vehicles or items stolen from vehicles.

Children must wait for their parents to pick them up inside the school. Please do not ask them to meet you elsewhere. Students are not to be dropped off on the street or curb. The FMS assumes no responsibility for children who leave unaccompanied. The students are not to be left at the school for extensive time periods and the school is not responsible to provide before or after care for students.

I understand that the information on this form will be entered into the FMS MindBody online scheduling software and will be available to the FMS front desk staff & teachers only; it is not available to the public and will never be sold. Any personal information can be erased upon request. I agree to receive email and text communication regarding appointment changes, updates, news, events, etc. Any communication can be terminated upon request at any time.

The card type/number can be changed/updated at any time before the 15th of the month. It is the student/parent’s responsibility to keep a current and valid card on file at all times while taking lessons at the school.

Program: Music FunTime is a comprehensive, progressive, structured curriculum that continues until student is ready for private music lessons in a particular instrument. At the discretion of the teacher, a parent or caregiver may be required to participate in class for 18 mos. to 3 yrs. of age. Children are taught music theory through a fun and innovated program with the use of instruments, teaching materials, crafts and singing. Students are given homework. When student is ready for private music lessons, Music FunTime teacher will advise parents/caregiver by suggesting a half hour private music lesson weekly.

Fees: There is a modest $25 registration fee per family each time you enroll. This covers your liability while on the school premises & some basic administrative costs. Tuition is $130 per month for 30-minute lessons, and $260 per month for 60-minute lessons. Payment is due on the first day of the month. Tuition is never prorated for missed lessons and does not cover any teaching materials such as books or instruments.

Average of 4 Lessons Per Month: Our calendar is designed to have an even number of 48 lessons per year, no matter which day of the week you choose. Most months you will have 4 lessons, sometimes 5, and sometimes 3 lessons - due to holidays. Monthly tuition stays the same. We do not charge you more when you have 5 lessons, or less when you have 3. This makes everyone’s accounting easier.

Class Time/Day Changes: If you need to change your class due to other commitments (going on with sports or other activities/interests) as always, we will try our best to accommodate you and find another time that works.

Snow Days: Forté will only close on Snow Emergency Level 3, and when that happens, we will make sure to post on our web site, social media, as well as send notifications via emails & texts. In order to make up the entire day that was missed, we try to open on the exact same day/time that the class was missed, during a scheduled vacation day such as Spring Break or Summer Break.

Spring Break: We are aware that the spring break vacation does not coincide with all schools. If your spring break does not coincide with Forté’s spring break, we will find a solution so you do not end up at loss.

Missed Classes: There are no credits or refunds given for missed classes, but you are welcome to attend another class on another day. Please be considerate and notify us in the event you are unable to attend your regular scheduled class.

Teacher Absences: We reserve the right to provide a replacement teacher if the regularly scheduled teacher is unable to teach any class (we will do our best to notify you in advance if a replacement will cover for your regular teacher). If a teacher is unavailable and we cannot provide a replacement teacher, the school will make a decision on when to schedule a makeup class, or the teacher will make arrangements to do a pre, or post make-up class.

Payments: All registered students are required to enroll in our auto-billing. This is a safe, secure, and convenient way to pay ongoing fees while enrolled at the school. We accept Visa, MasterCard, Discover, and American Express. We will automatically bill your card on the 1st day of every month. Our automated system will attempt to process the payment daily, for the first 7 days of the month. If the payment is still not successful after the 7th day, on the 8th day the charge will be placed on your virtual account with us, creating a negative balance that will need to be paid before your first lesson of the month. If payment is not received by the 2nd lesson of the month, you are subject to being removed from the schedule and losing your lesson spot.

Discontinuing Lessons: To discontinue lessons and to discontinue charges to your account, please fill out the withdrawal form before the 15th of your last month. We do not prorate tuition for the last month’s lessons. Students will be responsible for their last month’s tuition whether they choose to attend lessons or not.

Late Fee: In case the card on file is declined, we have a late payment fee policy of $20. If payment is not received within the first week of the month it is considered late and the fee will automatically be applied. It is the student’s/parent’s responsibility to ensure we have a current credit card on file for the auto billing.
